Starting March 3, 2020, low-income customers in Michigan can only qualify for Lifeline discounts if they qualify via the National Verifier. There are no Michigan-specific criteria as of that date. However, there are still different discount amounts based on Michigan regulations. They are listed here.
The forms below are being left as a reference for “grandfathered customers.” They should not be used for new customers or those who previously qualified via the National Verifier. Please contact us if you have any questions.
